Transcript A major accomplishment of the EIDP was the development of the study’s Common Protocol that addressed the research questions of the program. The Common Protocol along with instructions for administering can be downloaded from the University of Illinois’s Mental Health Service Research Program Web Site. We have provided you a link to this protocol and instructions in our course for your convenience. The link is located in this lesson with the title of EIDP Common Protocol. The Common Protocol consists of four major sections to include the Client Baseline Interview, the Client Six Month Interview, the Clinical Reporting Form, and the Employment Tracking Forms. The Baseline Interview is completed with each client as he or she enters the study. The Six Month Interview occurs six months from the Baseline Interview and assesses any changes since baseline as well as any aspects relevant to the client once services are initiated. The Clinical Reporting Form collects diagnostic information and information on alcohol and drug use. This form is completed outside of the interview setting at the beginning of the consumer’s study participation. Data is obtained through the case records and a clinician’s assessment. The Employment Tracking Forms are completed weekly by the staff member most knowledgeable of the consumer’s employment situation. The Client Baseline Interview collects information on Demographics, Residential Status, Finances and Entitlements, prior employment history, prior vocational training, work motivation, physical health, and current labor force status and work interests. In addition, there are scales to measure the individual’s quality of life and self esteem. The Client Level Six Month Assessment contains much of the information that was collected during the Baseline Interview. It also contains sections on employer disclosure, job satisfaction, and job impact as well as client satisfaction with services. The Employment Tracking Forms were developed to capture the complete work histories of the participants. Three forms were used to track the participants during employment to include the Employment Start Form, the Weekly Employment Change Form, and the Employment End Form. These forms allow staff to record and monitor the consumer’s progress on the job over time. The Employment Tracking Forms provide information on such things as salary, hours worked per week, level of workplace integration, description of the job for the Dictionary of Occupational Titles Coding, and disclosure and reasonable accommodations. |
